Membership Terms & Policies
Welcome to The Pannell Project! Please review our membership and package policies carefully before enrolling. By purchasing a membership or package, you agree to the following terms.
Scheduling & Appointments

Sessions are by appointment only.

You may schedule by online, text, call, or in person.

Please arrive 5–10 minutes early to get the most out of your session.
Cancellation & No-Show Policy

We kindly ask for at least 24 hours' notice if you need to cancel or reschedule your appointment.

You may cancel or reschedule by: Phone or Text: 662-205-8171 or Online: Via your scheduling link (provided after initial visit or request)

Appointments canceled with less than 24 hours’ notice may be subject to a $30 late cancellation fee.
A “no-show” occurs when a client misses an appointment without prior notice. In these cases:

The visit may be subject to a $50 no-show fee.

Repeat no-shows may lead to limited scheduling options or require prepayment.
Please note: NOT all Cards are not kept on file, and fees are not automatically charged. However, clients may be invoiced based on frequency and circumstances. Fee enforcement is at the discretion of the provider.
Payment Policy

The Pannell Project is a cash-based practice, which means we do not bill insurance directly.

However, we do accept credit cards, debit cards, checks, and cash.

HSA/FSA cards are not currently accepted.

If you’d like to file with your insurance independently, you may request a superbill before or after each session, which we are happy to provide.
Evaluation Requirement

A Free Consultation + Full Evaluation is required before starting any therapy, rehab, or mobility/strength services.
-
The full evaluation includes a movement screen, assessment, and a custom Home Exercise Program (HEP).
Add-On & A La Carte Services
-
These services can be booked without a full evaluation, but a free consultation is still required to determine if they’re a good fit.

No long-term commitment is required.
Discount Policies
-
High School & College Athletes receive 50% off all individual treatment sessions (excludes memberships and packages).
-
300 Fitness Members receive 25% off all treatment sessions and memberships.
-
Guest Fee: Non-300 Fitness members must pay a $5 guest fee per visit for Orthopedic Rehabilitation and Performance & Motion Analysis sessions.
Monthly Membership Plans

Plans auto-renew each month unless canceled with at least 7 days' notice before your next billing cycle.

Sessions within each plan must be used during that billing month and do not roll over.

Missed sessions may be rescheduled within the same billing cycle, if availability allows.
Youth & Athlete Services
-
Clients under the age of 18 must have a parent or legal guardian sign all consent forms and attend the first session.

In-season athlete care may include modalities such as cupping, IASTM, and E-Stim as needed.
Refunds & Transfers
-
There are no refunds for completed services.
-
Unused monthly sessions may not be transferred or rolled over unless pre-approved in writing.
Privacy & Documentation

Client records are kept confidential and compliant with HIPAA regulations.

We use secure digital documentation software (e.g., PT Everywhere) to manage assessments, treatment notes, and HEPs.
Risk & Client Responsibility

Participation in occupational therapy, physical therapy, rehab, or mobility and strength coaching involves inherent physical risk.

Clients are expected to inform their provider of any discomfort, limitations, or medical conditions.
-
The Pannell Project is not responsible for injuries sustained during unsupervised activity or failure to follow the recommended treatment plan.
